MINUTES July 5, 2001 3 BOARD OF HEALTH <br /> 5. Request for perk extension —61 Thornberry Circle <br /> The applicant is requesting an extension of the percolation test performed at 61 <br /> Thornberry Circle on July 23, 1998. Discussion took place regarding the soils <br /> at the site and the perc test timeframe. Two percolations tests have been <br /> performed on the lot in past years and have conflicting results. <br /> Mr. Ball motioned to deny the perc test extension at 61 Thornberry Circle. Mr. <br /> Santos seconded the motion. Motion to deny passed. <br /> 6. FYI — Copy of Phase II Stormwater Survey <br /> Board acknowledged. <br /> 7. Emergency Failure Certification <br /> 31 Oneida Avenue <br /> Health Consultant Charlotte Stiefel indicated that the leach pit is full to within <br /> 3" of the inlet and recommends a new distribution box and 3 500-gallon <br /> leaching chambers with 4' of stone. <br /> Board members forwarded their signatures to the emergency failure certification <br /> form. <br /> 29 Saddleback Road <br /> Ms. Warden indicated that the leach pit is full into the riser and that the <br /> installation of 2 500-gallon drywells with 4'of stone and a new distribution box <br /> is necessary. Request to waive camp permit fee "Circle of Caring" <br /> The applicant is requesting the waiver of camp permit fees for the ".Circle of <br /> Caring" non-profit recreational camp for children. <br /> Mr. Ball motioned to grant the fee waiver for the "Circle of Caring" camp for the <br /> 2001 season. Mr. Santos seconded the motion. Motion passed. <br /> 10. Letter from B. Wooldridge re: Smoking Ban <br /> Board acknowledged. Ms. Warden to follow up with a letter of <br /> acknowledgement. <br />
