OUTREACH COORDINATOR'S REPORT <br /> April 2011 <br /> HOME VISITS/APPOINTMENTS <br /> Twenty were made this past month, in addition to client contact by telephone. <br /> I met with several family members who are struggling with the care of an aging parent who is <br /> physically frail and/or experiencing significant memory loss. I referred these children to various <br /> resources such as certified and private home health/companion agencies, Elder Services of Cape <br /> Cod, Alzheimer's Association etc. In some of these cases I placed additional support such as a <br /> friendly visitor, library book delivery,transportation services, etc. Some of these caregivers are <br /> also experiencing their own challenges, such as mental health problems and/or lack of adequate <br /> finances. <br /> I continue to work with clients who are in desperate need of safe and affordable housing. In one <br /> situation, I arranged for the elder to stay in a local motel for several days,to diffuse a difficult <br /> family situation. <br /> VOLUNTEERS <br /> I continue to meet with the telefriend, SHINE, friendly visitor assistant, grief support, birthday <br /> flower delivery, and biography volunteers, to provide supervision, and to assign clients. <br /> I trained two new friendly visitors. <br /> I met with one of the two new SHINE counselors. The tentative plan is for the two new SHINE <br /> counselors to train with our current counselor, Antoinette Day during the month of May. <br /> Hopefully, the new counselors will be able to take appointments beginning in June. <br /> MEETINGS/CONTACTS <br /> I attended the monthly meeting of the town's Human Services Committee, and the monthly <br /> meeting of the Upper Cape Consortium. <br /> I met with two local therapists, Deborah Schwartz and Carol O'Connell, to discuss future <br /> collaborations. As a result, Deborah will be the facilitator/group leader for our new financial <br /> resiliency support group, beginning here on June 6t'. <br /> I met with Maureen Celorier, Clinical Liaison Spaulding Rehabilitation Hospital Cape Cod, to <br /> request community presentations on pain management and their driver evaluation program. <br /> I met with Ellen McDonough of Elder Services of Cape Cod, regarding the new Options <br /> Counseling Program. <br /> Respectfully Submitted, <br /> Lynne Waterman <br /> LFW/Icw <br />
