Glen Harrington remarked that the Board of Health Regulation states the distance to a well is to be a minimum <br />of 150 feet or greater. The Board cannot grant the well variance to 81 Lighthouse Lane because there is town <br />water available. There is no hardship when a better source of water is offered which is treated and monitored.. <br />Kalliope Egloff made a motion that the Board of Health approve the request for the Board of Health and Title V <br />septic upgrade variances for 81 Lighthouse Lane as follows: <br />1. A 4' variance, Soil Absorption System to property line (front), for a 6' setback. <br />2. A 3' variance, pump chamber to property line (side), for a 7' setback. <br />3. A 5' variance, Soil Absorption System to cellar wall, for a 15' setback. <br />4. A 2' variance to the 3' maximum cover requirement, for 5' of cover over the Soil Absorption System and <br />pump chamber. <br />The Board of Health denied the request for the 44' variance, Soil Absorption System to the well, for a 106' <br />setback for 81 Lighthouse Lane, because there is no hardship; therefore, no basis for granting the variance. <br />Burton Kaplan seconded the motion. The motion was unanimously approved. <br />Mr. McEntee will submit revised plans indicating the water service for 81 Lighthouse Lane. <br />NEW BUSINESS <br />1. Sign Expense Warrants. The Board members signed the expense warrants. <br />2. ReviewlApprove BOH Public Session Minutes: April 16, 2015. <br />Kalliope Egloff made a motion that the Board of Health approve the Public Session minutes of April 16, 2015, <br />as amended. Burton Kaplan seconded the motion. The motion was unanimously approved. <br />3. Request for 2015 Retail & Food Service Permit: Farm House Table (Kim Sweeney), 14 Central Sq. <br />Glen Harrington commented that Kim Sweeney, owner of Farm House Table LLC, submitted payment to <br />Eastern Food Safety for the Certified Food Manager's class and provided the Allergen Awareness invoice. Ms. <br />Sweeney has paid for the two permits, provided the menu, and has been inspected by the Health Agent. <br />Sandwiches, salads, and soups will be offered, and some of the desserts will be from Montilio's. Ms. Sweeney <br />is proposing outdoor seating. No major changes are intended. <br />Kalliope Egloff made a motion that the Board of Health approve Farm House Table LLC with the condition that <br />anything else not provided at this meeting needs to be submitted prior to the issuance of the permits. Burton <br />Kaplan seconded the motion. The motion was unanimously approved. <br />4. NSTAR Right of Way Herbicide Spraying of Electrical Easement: Comments to Selectmen. <br />Glen Harrington remarked that he has not been able to complete his comments for the NSTAR right of way. He <br />will finish the comments and forward a copy to the Town of Mashpee Selectmen and the Board of Health <br />members. <br />5. Bodywork Facility and Therapist Approval: Lyme Awareness of Cape Cod Entire Health & Wellness. <br />Glen Harrington stated that Lyme Awareness of Cape Cod was planning to have a medical doctor oversee all <br />medical work, but no doctor has been provided so the massage and Reiki therapist has requested a bodywork <br />permit and a facility permit to legalize their existence. Veronica Warden performed an inspection on April 21, <br />2015, and no violations were noted. <br />Kalliope Egloff made a motion that the Board of Health approve the request for Bodywork Practitioner and <br />Bodywork Facility, nonprofit, to Lyme Awareness of Cape Cod and Victoria Welch as presented.
