MINUTES March 8, 1990 3 BOARD of HEALTH <br /> 4. Accurate operational records shall he submitted to <br /> the Board of Health on a monthly basis. The <br /> records shall contain the following: <br /> I. Name of customer and address <br /> Distance to wetland <br /> . All private wells on the property <br /> 4# The amount of chemicals used on the job. <br /> APPOINTMENT: Cape & Islands - Title V Variance, lot 3 <br /> Wheelhouse lane <br /> Mr, John Sla inski, engineer for Cape & Islands, and Mr. <br /> Vince Porceillo, builder, were in attendance. Mr, Robert <br /> Lappin, abutter, was also in attendance. <br /> Mr. McQuaid informed the Board he is in receipt of three <br /> letters from abutters, Mr. Seymour Yanoff, Mr. Robert Lappin <br /> and Mr, David Edstrom, opposing the requested variances. <br /> Mr. Sla inski informed the Board in order to maximize <br /> the distance from the edge of the marsh the owner is request- <br /> ing a Title V Variance of I ' from the leaching facility to <br /> the road. No basement is proposed for the house. <br /> Mr. Lappin, abutter, stated he is opposed to the re- <br /> quested variances as it threatens the wildlife in the area. <br /> Mr, McQuaid informed the Board the lot will he serviced <br /> by High Wood Water, in conferring with. Cape & Islands the re- <br /> quested variance is proposed as an increase in the distance <br /> to the edge of the marsh is more favorable then reducing by <br /> lot to meet Title V. The Conservation commission has <br /> verified the edge of the wetland, however plan approval is <br /> contingent upon the location of the house and its proximity <br /> to the wetlands. <br /> J. <br /> Mr. Greelish stated placement of the leaching facility <br /> and dwelling are to marginal for the lot and motioned to deny <br /> local and Title V variances as requested, Mr. Doherty sec- <br /> onded, all agreed. <br /> APPOINTMENT: Names Gill Multivest - Bldg. permits for the <br /> "The Highlands" ,, formerly " amsgatel: <br /> Mr, Gill was in attendance for this scheduled appoint <br /> meat. <br /> Mr. McQuaid informed the Board the subdivision plan was <br /> approved by this Board with well and septic layout.
