%, MINUTES February 1 , 1989 3 BOARD of HEALTH <br /> Mr. O'Brien informed the Board that in order for the day-care to be <br /> re-licensed from the Office for Children they will be required to follow <br /> the regulations regarding lead paint. Mr. O'Brien explained that the Office <br /> for Children will issue a previsional license until such abatement takes <br /> place. Mr. O'Brien stated he has notified the person that was listed on <br /> the report,, Patricia Cann) , who informed him that she was in the process <br /> of obtaining estimates ,for de-leading. He explained that the office for <br /> Children will notify the parents of the children attending that day-care <br /> facility. <br /> APPOINTMENT: China Garden - Cede violations and license renewal <br /> Attorney Martin Cohen, representing Mr. Vincent Lai , Mr. Vincent Lai , <br /> County Health Agent Shaun O'Brien and Fields Point representative Melinda <br /> Gallant were in attendance for this appointment. <br /> The Barad provided copies of all documentation, regarding the restaurant <br /> to all persons in attendance. <br /> Mr. McQuaid stated on July , 1988 Mr. Lai was asked to meet- with the <br /> Board to discuss concerns and violations with the restaurant. Mr. McQuaid <br /> explained that he did offer his assistance with food preparation methods and <br /> area of food preparation in order to comply with State food sanitation regu- <br /> lations. Mr. McQuaid commented upon the second inspection vast improvements <br /> were made however upon the third inspection violations noted were identical <br /> to violations found on the first inspection. Mr. McQuaid explained, on the <br /> December 31 , 1989 inspection, Mr, Lai was informed of concerns and violations <br /> found,n , how er it was decided by himself to issue th permit, 1 . McQuaid <br /> stated he subsequently reviewed the past inspections and decided that the <br /> these violations and concerns should be reviewed by the Board prior to issu- <br /> ing the license. He commented upon the Board' s review it was decided to re- <br /> quest a meeting with Mr. Lai and partners of the establishment to discuss these <br /> violations and concerns. <br /> rt <br /> Mr. McQuaid presented a 'Written history of China Garden inspections <br /> acid vo i c'ed particular emphasis on the P.H.F. temperatures and food protection <br /> which did not show a decline. Th i s report is ,attached to the end of these minutes <br /> 'Mr. McQuaid stated areas of concern in relation to food protection <br /> were: food stored dirbctly on the floor i n the walk-ins, uncovered food in <br /> the walk-ins, the lack of thermometers in each ' refr i ger tion unit. Mr. McQu i ad <br /> explained the importance of sanitation and further elaborated that soap and <br /> towels were lacking in the bathrooms and wiping cloths for food contact ser- <br /> vices were not held -in a sanitizing solution, as well as, open storage of <br /> utensils. Mr. McQuaid discussed the problem with smoking in the kitchen and <br /> the unsanitary storage of linens and commented to Mr. Cohen and Mr. Lai that <br /> these violations are exmples that have been re- curing since operation o <br /> the estab I i shement began, Mr. McQuaid stated ceiling fans are blocked, holes <br /> in the kitchen ceiling, stove hoods rust be degreased, stairway is blocked <br /> with food, outside garbage has also been cited. <br />
