They still creed a motion to <br /> approve because it was continued last time. <br /> Mr. Santos made a motion to approve the 28'variance from the well for 35 So. <br /> Sandwich road with the condition of DEP approval. Request for New Body Art Operator's Permit: Thomas GriffinT� <br /> Mr. Doherty asked if Mr. Harrington stated that they could be here. <br /> Mr. Harrington stated that they were supposed to be here. However, he hadn't been <br /> in the facility yet. That was one of his comments. He did meet with them and <br /> requested a floor plan to review as part of the regulations. He stlll hasn't received <br /> it. <br /> Mr. Santos asked if they had a contract with a biohazard removal.company. <br /> Mr. Harrington hasn't reviewed all of those items with the applicants. He was <br /> looking at this again to make recommendations to the BOH members. All of the <br /> items on his list are part of the regulations. He should be going through these one <br /> by one with the applicants, They should be handing theca to the BOH. He gave <br /> there the regulations. They have not turned these items back over to him and until <br /> they can prove to the BOH that they have these items. He is not going to approve <br /> this request. <br /> Mr. Doherty stated that one thing that just jumped of the page on the application <br /> for license to practice body is the previous employer item. There was no name or <br /> address or way to contact this man. He felt that there should be more area to add <br /> pertinent information about the employer to follow up on his employment history. <br /> Mr. Harrington stated that there was a letter in there from his employer. <br /> Mr. Doherty was stating that his point was that without the letter from the prior <br /> employer. It would be next to impossible to track down this employer, <br /> Mr. Santos stated that the body art operator worked in New Hampshire. Does the <br /> state require a permit there? <br /> 1 <br />
