r <br /> 8:00 PM Uncommon cafe—2008 Food Establishment Permit <br /> Mr. Shaven Boynton, owner of the Uncommon Caf , introduced himself to the Board. <br /> In a letter to Mr. Boynton from the Board of Health, dated .July 17, 2008, it was acted that <br /> Mr. Boynton had not applied for or received a 2008 Food Establishment Permit to <br /> operate a food establishment. The letter states that Mr. Boynton purchased-the <br /> Uncommon Caf& located at 14 Central Square in Mashpee on May 15, 2008 and has <br /> been operating since that date. Because Mr. Boynton failed to obtain a permit to <br /> operate a food establishment, he was in violation of the State Sanitary Code for Food <br /> Establishments, and the letter issued a cease and Desist order by the BOH. The order <br /> stated that the establishment shall remain closed until a Food Establishment Permit <br /> application has been reviewed by the BOIL on 7128108 at 8 PIIS. The order also revoked <br /> the previously approved The Uncommon Caf&temporary Food Establishment Permit for <br /> the 7118108 Mashpee commons event. <br /> Mr. Boynton stated that he had never been in the restaurant business before. He stated <br /> that his brother, who will be working in the restaurant, is Sery Safe Certified, and that <br /> both he and his wife will be getting their Sery Safe certification. Mr. Boynton stated that <br /> when he applied for, and received a temporary Food Establishment Permit for the,duly <br /> 18, 2008 "Moce n" event, no one at the BOLI questioned him as to whether he had a <br /> 2008 Food Establishment Perm t. Mr. Boynton, in effect, was pleading ignorance, <br /> stating that he thought everything was in order when he purchased the business in May <br /> of 2008. <br /> Burton Kaplan made a motion to approve the Food Establishment Permit application for <br /> The Uncommon Caf , pending BOH inspection of the premises prior to opening, and <br /> receipt of a copy of the menu. Kalliope Egloff seconded the motion. All approved. <br /> Lucy Burton made a motion to approve the temporary Food Establishment Permit <br /> submitted by The Uncommon Cafe for the Mashpee Commons Pops Concert on <br /> 7/28/08. I alliope Egloff seconded the motion. All approved. <br /> NEW BUSINESS <br /> 1i, Sign Warrants. There were no warrants. <br /> 2# Approve Public session BOH Minutes of 7109108 <br /> The following corrections were rade to the 7109108 BOH Minutes.. <br /> Page 1, under APPOI NTME NTS 7:15 PM, Draft Food services Regulation, <br /> paragraph 1, line 10, delete the words preparation of the food and insert removal <br /> from temperature control <br /> - Page 2, thin paragraph, correct the spelling of the name Stillman to Silliman <br /> throughout the paragraph <br /> Fuge 3, under 7:30 PM-John Bei e1l Tinder Box, paragraph 8, line 2, place a <br /> period after the word cleanup and delete the rest of the sentence. <br /> Page 3, last paragraph, line 4, add the word and before the word ultra. <br /> Page 5, under 6. Request for Review and signature of Landfill Monitoring <br /> Contract, change the spelling bf the name lloodarf to Woodard. <br /> Page 5, under OLD BUSINESS, 2, Lakeside Estates, lines 2 and 5, change the <br /> spelling of the name Kurrane to Kirraine. <br /> Page 5, under DISCUSSION, paragraph 1, line 3, change Ct. to County. <br />
