Map C (66va cc OShap 18SA,rhumet Qoad (:A(xhpx,, NCA 026,f9 F10807r73990 April 23, 2009 Town of Mashpee Plan Review Committee 16 Great Neck Road North Mashpee, MA 02649 Respected Members of Plan Review Committee: With regard to the proposed kennel to be located at 225 Ashumet Road, we have put together some conditions to be placed on this license should you accept this application. 1. No more than six (6) dogs allowed in this kennel at any time including puppies 2. This kennel shall be built with an 8 foot chain link fence around it trenched into the earth a minimum of 2 feet anchored in concrete. 3. The lot shall be then enclosed with an eight (8) foot tail stockade fence around the entire perimeter. 4. No more than 2 dogs are to be out of the kennel at any time. 5. When the dogs are out of the kennel a muzzle is mandated. 6 . As the pit bulls pass on, they may not be replaced until the quantity is no more than 3. 7. An insurance certificate is required through a surplus line of insurance such as Lloyds of London or Scottsdale etc., that states the mortgage company and the insurance company are aware that there are 6 pit bulls on the property along with a kennel and that each dog is insured for a minimum of $1,000,000/$2,000,000 total aggregate liability and medical cost coverage should one or more of these pit bulls escape and cause physical harm or death. 8. This insurance certificate shall be forwarded to the Clerks office on a quarterly basis (4 times a year) to prevent cancellation of the policy. Failure to provide certificate of insurance to the Town will result in immediate confiscation of said Pit bulls and revocation of kennel license. 9. Inspections can be performed at any time without prior notice to the owners by either Police, Zoning, or Board of Health as deemed necessary to ensure proper health and containment and dog quantity. 10. All pit bulls are to be either spayed or neutered. 11. No breeding of these pit bulls may be allowed. 12. To ensure that the applicants/owners of these pit bulls are responsible members of the community, a favorable CORI must be presented by both applicants. Should a criminal history be revealed by this CORI on either applicant, this kennel must be denied.
