5 <br /> f <br /> Laurel Almquist made a motion that the Board of Health approve the 2018 Food Establishment Permit for <br /> Falcon cafe at Mashpee High School. Mallory Langler seconded the notion. The motion was unanimously <br /> approved. <br /> 3. Request for 2018 Food service Permit: The chocolate Rose (628 Main street). = <br /> Glen Harrington commented that Ms. Benoit is an ownerloperator of a baking and catering business, the <br /> Chocolate Rose, where she would like to expand to a larger operating space. On November 30, 2017, the <br /> Board approved a variance for the facility to utilize a grease recovery device in lieu of an in-ground grease trap. <br /> The bakery and catering permit application, menu, serve-safe certificate and allergy awareness certificate have <br /> been submitted along with the fee. The final floor plans have also been submitted and reviewed. <br /> This property is serviced by an on-site potable well. Per the Department of Environmental Protection guidance: <br /> the food establishment must utilize bottled water for preparing food. No seating is allowed due to the fact that if <br /> the seated patrons utilize the bathrooms, the well will be considered a public supply well which it cannot as the <br /> protective radius is not all on the subject property. <br /> Vanessa Benoit, owner of the chocolate Rose, introduced herself to the Board. FIs. Benoit remarked that the <br /> issues with the grease recovery device have been resolved and will be installed next week. <br /> Mallory Langler made a motion that the Board of Health approve the food establishment permit with the <br /> following conditions: <br /> 1. Bottled water be utilized to prepare all foods. <br /> . No seating for public patrons be provided until torn water is provided to the property and <br /> establishment, <br /> . Request for 2018 Food Service Permit and Tanning License (change of Ownership): The Fit <br /> Company for Women (502 blain street). <br /> Glen Harrington commented that the Fitness Company for Women has had a change of ownership. The neer <br /> owner has requested a food establishment permit and tanning permit to operate at the facility. The owner does <br /> not have her certified food manager training yet but is in the process of obtaining it, The Board has the option <br /> to allow up to 60 days for the owner to obtain the certified Food Manager's certificate, or only allow the sale of <br /> pre-packaged foods until the owner obtains the certificate, or issue the food establishment permit after an <br /> inspection is performed for the preparation. The application for the tanning permit has been submitted, and the <br /> fee has been paid. <br /> Mallory Lander made a motion that the Board of Health approve the Food Establishment Permit for the Fit <br /> Company for Women with the following conditions: <br /> 1. The Fit company is required to obtain a certified Food Manager's certification within 60 days of <br /> obtaining the permit. They will be allowed retail sale of pre-packaged food until the certification is <br /> obtained. <br /> . Inspection by a Health Agent is required prior to issuance of the food permit. <br /> . Approve the tanning permit as presented. <br /> Laurel Almquist seconded the motion. The motion was unanimously approved. <br /> 5. Variance Request to cesspool Regulation: 76 Sum ersea Road. <br /> Glen Harrington stated that the current owner, Donald Reynolds, has submitted a request for a variance to sell <br /> the property at 76 Surnmersea Road without upgrading the existing cesspools in lieu of the requirements of the <br /> Cesspool Regulation. The buyer, James Hirbourx was contacted to attend the public greeting to discuss the <br /> conditions of the enforceable agreement that will mitigate the variance. <br /> 2 <br />
