7:20 PM W Nitrogen Aggregation Loading Plan Request— 14 Strawberry Lane. <br /> Glen Harrington stated that this request consists of a nitrogen aggregation plan for 14 Strawberry Lane which <br /> requires 32,000 square feet of land as credit land to attain the 40,000 square-foot threshold and 4 bedrooms. <br /> The Grant of Title V Easement and Restrictions have been prepared for the Facility and Credit land in draft <br /> form. The septic system design plan is not included with this packet and must be approved at a later date with <br /> a compliant Title V design plan. The perk tests have been performed, and the nitrogen aggregation plan review <br /> fee has been paid. <br /> Matt Costa, Cape and Islands Engineering, representing the applicant of 14 Strawberry Lane, introduced <br /> himself to the Board. <br /> Mr. Costa explained the procedure of a Nitrogen Aggregation Plan for Laurel Almquist so that she can become <br /> acquainted with the process. <br /> Laurel Almquist made a motion that the Board of Health approve the Nitrogen Aggregation Plan for 14 <br /> Strawberry Lane with the condition that the Grant of Title V Easement and Restrictions for the Facility and <br /> Credit-land be recorded at the Registry of Deeds with copies provided to the Board of Health prior to issuance <br /> of a Building Permit and also Matt Costa is to submit a revised Zone II Map to the Board of Health. Brian <br /> Baumgaertel seconded the motion. The motion was unanimously approved. <br /> OLD BUSINESS <br /> 1. Review/Approve BOH Public Session Minutes: April 5, 2015. <br /> Laurel Almquist made a motion that the Board of Health approve the Public Session minutes of April 5, 2018. <br /> Brian Baumgaertel seconded the motion. The motion was unanimously approved. <br /> NEW BUSINESS <br /> 1. Sign Expense Warrants. The Board members signed the expense warrant. <br /> 2. Bathing Beach Variance: Seconsett Island Causeway Beach. <br /> Glen Harrington commented that the Department of Public Health is requesting that the Board of Health <br /> approve a variance from sampling weekly to monthly for the Seconsett Island Causeway Bathing Beach. A <br /> sanitary survey performed by the Department of Public Health states that the beach has not had a closure in <br /> four years which makes it eligible for reduced sampling. The Board of Health had approved the same variance <br /> for Callies Beach in 2007. At first, the Department of Public Health requested that the testing be eliminated <br /> because swimming should not be allowed where boating traffic is heavy. Veronica Warden stated that the <br /> beach is used by bathers and beach-goers too often to eliminate the testing totally. Mr. Harrington is <br /> requesting the sampling be performed monthly rather than weekly. <br /> Laurel Almquist made a motion that the Board of Health approve the variance as presented with a sampling <br /> frequency of once prior to the bathing season and every 30 days thereafter during the bathing season in lieu of <br /> the requirements of 105 CMR 445.032(C)(1) (a & b). Brian Baumgaertel seconded the motion. The motion was <br /> unanimously approved. <br /> 3. Requesting One-Time Fee for Annual Temporary Food Permit: Chariot Pizza (Brian Shields). <br /> Glen Harrington stated that Brian Shields, owner of Chariot Pizza, has requested a one-time fee for attending <br /> events throughout the year. The food operator does not have a mobile food cart or trailer but sets up a tent <br /> with the wood-fired pizza oven. The owner works out of a licensed facility in Bourne. The Catering Permit was <br /> included with the submittal of the application. <br /> Laurel Almquist made a motion that the Board of Health approve the one-time, annual, temporary food permit <br /> fee waiver for Chariot Pizza with the condition that the operator comply with all federal, state, and local <br /> temporary food requirements.
