A doctor ' s certificate at the expense of the employee may <br /> be required by the Department Dead or the Personnel Board if <br /> an employee is absent from work for three consecutive <br /> days or more, or for ten l or more days in a twelve 12 <br /> month period, or if the Personnel Board or Department Head <br /> has warred the employee about abuse of sick leave privileges . <br /> el A non-probationary employee, injured in the performance <br /> of duty, may use accumulated sick leave to make up the dif- <br /> ference between workmen ' s compensation benefits received by <br /> the employee and the employee ' s base pay. <br /> f. An employee may use up to three d days per year of his <br /> sick leave time for leave to tape care of a member of his/her <br /> family living in the same household who is sick, <br /> g. Sick Leave Bark , All persons covered by the Plan at their <br /> option may contribute from their personal sick leave accumu- ' <br /> lations into a sick leave bank as follows : Each employee may <br /> make an initial contribution of two 2 sick days upon com- <br /> pletion of his probationary period, ( Incumbent employees who <br /> have already completed their probation may also make this <br /> contribution. ) In each subsegment year, in the month of July, <br /> employees may contribute one l sick day until the bank con- <br /> tains a total minimum of sixty fro days . Employees who have <br /> exhausted their personal sick leave credits may apply to the <br /> Board for a grant of bank days in cases of extended Illness . <br /> The Board in its discretion shall decide whether or not to <br /> grant the bank days requested taking into account the employ- <br /> ees sick leave history, The Board ' s decision shall be final <br /> and not subject to the grievance procedure set out in this <br />
