PaolettaG& Hoit, <br /> Each of you are cited for the following violations of 780 CMH The State Building Code as <br /> indicated: <br /> 1. Door L 20- Floor at door is uneven and a tripping hazard. D'Agostino <br /> 2. Mechanical morn, lower level -seal pipe openings in exterior wall. water line penetrations <br /> leak and a wall patch is leaking above and into sprinkler system notifr TS- . D'Agostino <br /> 3. Mechanical room, lower level -air handler support, I -D wood in contact with concrete. <br /> D'Agostino <br /> 4. Stair#1 pump room-temporary light in pump room. 780 CMR Article 27 -D°Agostino <br /> . Stair 9 l pump room-no handle or lock on door. D'Agostino <br /> . 780 CMR Article 27 - Electrical -all sub-panels are missing covers and the majority are <br /> missing blank plates. D'Agostino <br /> 7. 780 CMR Article 2 - Mechanical Equipment-excessive diffuser& exhaust noise, Building, <br /> conference roorns, etc. - faulty design or areas are not prope r y balanced. Hcat control <br /> problem- faculty design or balance. Filters are not missing in Air Handler#1. D'Agostino <br /> . As-built drawings, operation instruction sets(2),Manufacturer Brochures have not been <br /> submitted. Engineer must certify systems and their proper operation it appears their operation <br /> is faulted). This is required for all systems(plumbing, gas,electrical, HVAC,etc.)_ <br /> D'Agostino <br /> . Drawing A 1. , D eta i 19 1,23 - Project Manual, Section 073'10, Part 2 -This section call <br /> for the removal of the existing shingles and the gutters which were not done. The gutters were <br /> lined and the new shingles installed over the old.The non-compliant work is to removed and <br /> the shingles and gutters are to be installed per contract documents. D'A ostino-780 CMR <br /> Section 127.3 ite m 1 execute work according to construction documents , item 2 (failure to <br /> note all pertinent deviations specifically). <br /> 10. 780 C M R Section 1110.4 - Roof structure over executive secretary does not meet ponding <br /> regulation for structural loads. Scupper required.D'Agostino <br /> 11. A 1.5 Detail 1 - Due to numerous Maks it is presumed thxu-wall Bashing was not installed at <br /> the elevator wall nor were weep holes created. D'Agostino <br /> 12. A2.1 - New wood trim required for the outside of the elevator shaft and stair 92 has not been <br /> properly installed and flashed. D'Agostino <br /> 1 . Numerous leaks - leisure services, clerks,administrative secretary, treasurer, roof on 1920 <br /> building and especially in the newly built section. D'Agostino <br /> 14. Ceiling the missing in leisure services. D'Agostino <br /> Strekalo sky& Hoit- items#7 & 8 -780 CMR 127.2.3 (issuance of a substantial completion with <br /> non-compliance of acceptable practices, item s 9& 11 -780 C MR Section 127 2.2 failure to insure the <br /> work has proceeded in accordance with the documents approved for the building permit and failure to issue <br /> a corrective directive)and 127.2.3 failure to Inst pertinent deviations for the approved building <br /> documents , item 10 - 780 CMR 127.2.1 (plans do not meet the applicable provisions of this code)and <br /> 127.2.3 (issuance of a substantial completion without noting a code violation) <br />
