§84-4 PLAN AND APPLICATION REQUIREMENTS A. The following information shall be included on the Erosion and Sedimentation Control Plan and/or included with the application: (1) fame, address, and telephone number of owner, civil engineer, and person. responsible for implementation of the plan, (2) Street address and Assessors' map and block number of property; (3) Description of the property, including a mets and bounds description of all boundaries or property lies; (4) Location of all existing and proposed structures and impervious surfaces; (5) Location of all existing and proposed storm water facilities, including structures, pipes, swales and detention basins; (6) A vicinity map showing the location of the site in relationship to the surrounding area's watercourses, water bodies, and other significant geographic features, including all ways and significant structures; (7) Minimum 2 foot contours for the existing and proposed topography, using the North American vertical. Datum of 19 (NAVD88); (8) A clear delineation of any areas of vegetation or trees identifying all vegetation that is to be removed and all vegetation that is to be preserved; (9) A clear delineation of any wetlands as defined by Chapter 1.72 of the 1' ashpee General Bylaws, any natural or artificial water storage detention areas, and any drainage ditches on the site or within 100 feet of the property lines; 1 of esign details for both temporary and permanent erosion control structures; 11 Descri Lion of all erosion and sediment control measures proposed to minimize on-site erosion and prevent off-site sediment transport, including provisions to preserve topsoil and limit disturbance; (121nformation on seeding mixtures and rate, type of sod, method of seedbed preparation, type and rate of fertilizer application, and type of mulching for both temporary and permanent erosion control measures. Seed mixtures and/or sod shall not contain more than ten percent l 0%) Kentucky Blue Grass.; and (13)A sequence of activities for development of the site, including stripping and clearing, rough grading, construction of utilities, infrastructure, and buildings, and final grading and landscaping. 9
