§84-4 PLAN AND APPLICATION REQUIREMENTS <br /> A. The following information shall be included on the Erosion and Sedimentation <br /> Control Plan and/or included with the application: <br /> (1) fame, address, and telephone number of owner, civil engineer, and person. <br /> responsible for implementation of the plan, <br /> (2) Street address and Assessors' map and block number of property; <br /> (3) Description of the property, including a mets and bounds description of all <br /> boundaries or property lies; <br /> (4) Location of all existing and proposed structures and impervious surfaces; <br /> (5) Location of all existing and proposed storm water facilities, including structures, <br /> pipes, swales and detention basins; <br /> (6) A vicinity map showing the location of the site in relationship to the surrounding <br /> area's watercourses, water bodies, and other significant geographic features, <br /> including all ways and significant structures; <br /> (7) Minimum 2 foot contours for the existing and proposed topography, using the <br /> North American vertical. Datum of 19 (NAVD88); <br /> (8) A clear delineation of any areas of vegetation or trees identifying all vegetation <br /> that is to be removed and all vegetation that is to be preserved; <br /> (9) A clear delineation of any wetlands as defined by Chapter 1.72 of the 1' ashpee <br /> General Bylaws, any natural or artificial water storage detention areas, and any <br /> drainage ditches on the site or within 100 feet of the property lines; <br /> 1 of esign details for both temporary and permanent erosion control structures; <br /> 11 Descri Lion of all erosion and sediment control measures proposed to minimize <br /> on-site erosion and prevent off-site sediment transport, including provisions to <br /> preserve topsoil and limit disturbance; <br /> (121nformation on seeding mixtures and rate, type of sod, method of seedbed <br /> preparation, type and rate of fertilizer application, and type of mulching for both <br /> temporary and permanent erosion control measures. Seed mixtures and/or sod <br /> shall not contain more than ten percent l 0%) Kentucky Blue Grass.; and <br /> (13)A sequence of activities for development of the site, including stripping and <br /> clearing, rough grading, construction of utilities, infrastructure, and buildings, and <br /> final grading and landscaping. <br /> 9 <br />
