" <br /> "'Mr. Evans certified the system in failure and directed the <br /> installer to proceed immediately subsequently, the Board <br /> signed the emergency repair certification. " "Last week I was <br /> contacted for an inspection. " "At that time I noticed the <br /> proximity of the bank. " "I questioned the installer as to <br /> whether the Conservation Commission was notified. " "The in- <br /> staller stated he was unsure. " "I did inform the Conserva- <br /> tion Commission agent who inspected the site and informed the <br /> installer he had to file a notice of intent. " "We should <br /> have in retrospect notified the Conservation Commission ahead <br /> of time however, we did not. " "In my opinion the repair was <br /> more important than the paper work. " "I would recommend the <br /> Board direct the agent to forward a letter to the Conserva- <br /> tion Commission explaining the circumstances.
