MINUTES September 22, 1994 2 BOARD OF HEALTH <br /> Mr. McQuaid relayed, "I would recommend we honor our soil logs agreement and <br /> submission of Title V plans for an upgrade of all the remaining systems." "If they are filed <br /> between now and the end of this year they will be grandfathered against the revised Title <br /> V requirements on the condition that Mr. Haney follow-up with his agreement that he will <br /> upgrade five a year." <br /> Board members agreed. <br /> Mr, McQuaid stated, "I will respond accordingly." <br /> 4.) Request for fee waiver, Habitat of Cape Cod - Mr. McQuaid stated, "I am <br /> involved in this Habitat of Cape Cod through my church." "One of our church trustees is <br /> a trustee of Habitat of Cape Cod." "They were given a piece of property in Mashpee and <br /> have asked me to request Board members waive the well, pert and septic permit fees as <br /> they are a non-profit organization." "The dwelling will be ]eased out to a low income <br /> family." <br /> Mr. Doherty stated, "We need to specify the location of the lot and we will require <br /> proof of non-profit status." <br /> Mr. Cram motioned to approve the request to waive the perc, well and septic <br /> permit fees with the stipulation the agent is informed of the location of the lot and Habitat <br /> of Cape Cod provide proof off non-profit status, Mr. Doherty seconded. <br /> APPOINTMENT - Cape & Islands - 19 Tide Run (Bio-Clere) <br /> Mr. John Slavinsky, P.E. was in attendance for this scheduled appointment <br /> Mr. McQuaid relayed, "The Board approved the system irrespective of the Bio- <br /> Clere." "Subsequently, the owner revised plans with a Bio-Clere system which the Board <br /> approved conditional upon D.E.P. concurrence." <br /> Mr. Slavinsky stated, "I am requesting the Board provide a letter stating the <br /> permits were previously issued without the Bio-Clere and that the Board has no problem <br /> adding the Bio-Clere to this particular lot." <br /> Board members agreed to provide the letter as requested by Mr. Slavinsky. <br /> 5.) Response to Ms. Jones re: Recycling operation - Acknowledged. <br />
