MINUTES April 13, 2000 6 BOARD OF HEALTH <br /> existing well to the leaching pit of#829. Applicant is also proposing the <br /> installation of a Micro-Fast Denitrification system to allow for a 2-bedroom <br /> home in a Zone II area. <br /> Mr. Doherty motioned to grant the variances as outlined in a cover letter from <br /> Cape & Islands Engineering dated September 7, 2000. Mr. Santos seconded <br /> the motion. Motion passed. <br /> 67 Riverside Road <br /> The applicant-has submitted revised plans due to the Conservation Commission <br /> request that an existing sassafras tree be saved. The revised plans would now <br /> require a variance of 5' from the proposed soil absorption system to the road <br /> layout line. <br /> I <br /> Board members noted that the plans have been submitted without a locus as <br /> required by Board of Health policy. <br /> i <br /> Mr. Doherty motioned to approve the variance as outlined in a cover letter from <br /> BSS Design dated September 18, 2000. Mr. Santos seconded the motion. <br /> Motion passed. <br /> 12. Emergency Failure Certifications <br /> 222 Fells Pond Road <br /> Mr. McQuaid indicated that the cesspool is full and a new shallow pit should be <br /> installed off the second tank. <br /> Board members forwarded their signatures to the Emergency Failure <br /> Certification form. <br /> I <br /> 130 Captains Row <br /> Mr. McQuaid indicated that the septic tank was overflowing onto the ground. <br /> Board members forwarded their signatures to the Emergency Failure <br /> Certification form. <br /> 21 Edgewater Road <br /> Mr. McQuaid indicated that the leach pit was full to overflowing and that a new <br /> pit 100' away from the well should be installed. <br /> I <br /> Board members forwarded their signatures to the Emergency Failure <br /> I <br /> Certification form. <br /> 8 Essex Road <br /> Ms. Warden stated that the leach pit is full. Installation of a new pit as per <br /> plan (new d-box and "t" required also) is necessary. <br /> Board members forwarded their signatures to the Emergency Failure <br /> Certification form. <br />
