6.4.4.6 If in any month building permits for fewer dwelling units are <br /> issued than are allowed by the limits of Subsection 6.4.2.2, <br /> the difference between the limit and the actual number of <br /> dwelling units for which permits were issued may be added to <br /> the limit for the following month, except at the end of the <br /> calendar year. <br /> 6.4.4.7 Where a building permit is to expire, no more than one (1 ) <br /> six-month extension may be granted by the Building Inspector <br /> before an applicant shall be- required to file for a new permit <br /> following the rules and procedures of Section 6.4 and other <br /> i applicable laws and regulations. <br /> 6.5 WATER QUALITY REPORT <br /> 6.5.1 As a part of any application for approval of a Definitive Sub- <br /> division Plan (excluding plans which would divide any parcel <br /> shown on the 1985 Mashpee Assessors' Maps into 5 or fewer lots <br /> or where proposed density of residential development would be <br /> less than one unit per two acres) or of a Special -Permit requir- <br /> ing approval by the Planning Board, the applicant shall submit <br /> ten (10) copies of a Water Quality Report regarding the proposed <br /> development. Said report shall be completed by a qualified <br /> firm, experienced in groundwater and surface water quality <br /> evaluation and in determining the impacts of land uses on <br /> receiving waters. One copy of the Report shall be transmitted <br /> for review and comment to the Board of Health. <br /> 6.5.2 The Water Quality Report shall contain, at a minimum, the. <br /> following information: <br /> (a) Whether or not the development lies within a Groundwater <br /> Protection District (see Section 11 ) , within a previous- <br /> ly defined "Zone II" recharge area of an existing or pro- <br /> posed public water supply well , within 300 feet of the <br /> Santuit, Mashpee, Quashnet or Childs Rivers, Quaker Run <br /> south of Route 28, Red Brook, or any pond, bay or other <br /> surface water body or any adjacent wetlands as defined <br /> by Massachusetts General Law Chapter 131 , Section 40, or <br /> within the ground water recharge zone of any great pond or <br /> bay or other surface water body over one acre. (With re- <br /> gard to the groundwater recharge zones of Santuit Pond, <br /> Mashpee-Wakeby Pond, Johns Pond and Moody Pond, Jehu Pond, <br /> Ockway Bay and Hamblin Pond/Red Brook, reference shall be <br /> made to maps produced by K-V Associates of Falmouth, Mass. <br /> which are on file in the office of the Town Planner. For <br /> other great ponds or bays and surface water bodies over <br /> one acre a map of the groundwater recharge zone shall <br /> 'I <br /> be produced as part of the Water Quality Report.) <br /> (b) Whether or not the development will produce any hazardous <br /> or toxic materials (as defined in 314 CMR Section 3.02 <br /> (21 ) , (22) and (48)) or if such materials will be stored <br /> 6 - 20 <br />
