IN <br /> The following items shall be submitted to each <br /> board at least fourteen ( 14 ) days prior to each <br /> pre-application conference and to the Town Planner, <br /> the Superintendent of Streets , the Fire Chief , <br /> Police Chief and the consulting engineer to the <br /> Planning Board at least fourteen ( 14 ) days prior to <br /> the first of such conferences : <br /> (a ) A topographic plan of the entire site , <br /> including all parcels proposed for open space <br /> or for development , showing topography at five <br /> ( 5 ) foot contour intervals or less , all <br /> existing paved and improved roads , ways and <br /> trails and all existing structures . <br /> ( b ) A natural resource map or maps , on a copy or <br /> copies of the topographic plan , indicating all <br /> wetlands as defined by M. G. L. Chapter- 131 , <br /> Section 40 or otherwise referenced in this <br /> Section , generalized vegetation types and <br /> location , soil types based on the latest <br /> information available from the USDA Soil <br /> Conservation Service or on more detailed <br /> information obtained by the applicant , and <br /> depth to ground water table based on <br /> exploration by the developer or on other <br /> reliable data acceptable to the Board of <br /> Health . <br /> ( c ) A sketch plan of the proposed development <br /> showing the approximate boundaries of areas <br /> proposed as open space as required by <br /> Subsection 9 . 3 . 3 . 1 , the general location of <br /> streets referenced in Subsection 9 . 3 . 5 . 6 ( b ) , <br /> the general location of any clusters or areas <br /> of residential development , the location of <br /> any wellfield or sewage treatment facility , <br /> and the general location of any recreation <br /> facilities , commercial structures or other <br /> major features of the development . <br /> 9 . 3 . 7 . 2 Special Permit Application - After pre- <br /> application conferences have been completed , the <br /> applicant may submit an application ( containing all <br /> of the items required below ) for a special permit <br /> for an Open Space Incentive Development to the <br /> Planning Board at a regularly scheduled meeting , a <br /> copy of which shall forthwith be given to the Town <br /> Clerk by the applicant . Within sixty-five ( 65 ) <br /> days but no less than forty (40 ) days after the <br /> filing of such application with the Board , the <br /> Board shall hold a public hearing for which proper <br /> notice has been given by publication or posting as <br /> required by. M. G. L . Chapter 40A Section 11 , and -by <br /> 9 - 16 <br />
