mailing to all parties in interest , including the <br /> owners of all real property within three hundred <br /> (300 ) feet of such development. <br /> The following items shall be required as part of <br /> the application for a special permit under this <br /> section , along with receipts indicating that items <br /> (c ) through (h ) and item (k ) have been submitted to <br /> the Board of Health , Design Review Committee , <br /> Conservation Commission , Town Planner , <br /> Superintendent of Streets , Fire Chief , Police Chief <br /> and Parks and Recreation Director : <br /> (a ) Any required fees established by the Planning <br /> Board along with any application forms <br /> established by the Board for the purpose , <br /> signed by the owner or owners of the parcel or <br /> parcels of land to be included in the OSID. If <br /> the application is not signed by the owner, it <br /> shall be accompanied by a notarized letter <br /> signed by the property owner authorizing the <br /> agent signing the application to act in his <br /> behalf in all matters relative to the <br /> application and to legally bind the owner <br /> regarding any conditions or requirements of <br /> the special permit . More than one party may <br /> be named as an applicant fora special permit . <br /> However , the special permit is not <br /> transferable to other parties not named 'in the <br /> application without specific written <br /> permission from the Planning Board . <br /> (b ) A copy of the latest recorded deed or deeds to <br /> all property included within the proposed OSID <br /> along with a copy of any currently valid <br /> recorded subdivision plan or special permit <br /> applicable to the site of the proposed OSID. <br /> (c ) A master site plan showing existing and <br /> proposed topography at two (2 ) foot intervals , <br /> all proposed roads as referenced in Subsection <br /> 9 . 3 . 5 . 6 (b ) with plan and profile drawings as <br /> required by the Board for definitive <br /> subdivision plans , and all existing paved and <br /> unpaved roads , ways and trails to remain or <br /> proposed to be moved or removed , proposed <br /> general locations for structures other than <br /> detached single-family residences on <br /> subdivision lots , the location of any proposed <br /> recreational facilities or other amenities , <br /> the location of any wetlands as heretofore <br /> defined , the- general boundary of any areas to <br /> be subdivided into single-family residence <br /> lots showing the approximate number of such <br /> 9 - 17 <br />
