lots proposed and the location of future road <br /> access points from such subdivision onto the <br /> roads referenced in Subsection 9 . 3 . 5 .6 (b ) and <br /> required to be shown on this plan , the items <br /> required by Subsection 9 . 3 . 6 to show any <br /> proposed phasing of the development , the <br /> general location of any bikeways or pedestrian <br /> facilities and the specific location of the <br /> surveyed and bounded areas to be set aside as <br /> open space in conformance with Subsection <br /> 9 . 3 . 3 . ( Scale and . size of plans etc . shall <br /> conform with the Planning Board regulations <br /> for definitive subdivision plans unless a <br /> waiver is granted by the Board prior to the <br /> submission of the Special Permit application ) . <br /> (d ) Accurate calculations of the area within the <br /> OSID of wetlands , of open space to be <br /> preserved or transferred to the Town or to a <br /> non-profit organization , and of areas proposed <br /> for roadways and other development along with <br /> the applicant ' s calculation of allowed <br /> dwelling units based on the provisions of <br /> Subsection 9 .3 . 4. <br /> ( e ) The developer ' s declaration of his choice of <br /> method or methods of dedicating the required <br /> open space of the three methods described in <br /> Subsection 9 . 3 . 3 .4 , along with proposed <br /> covenants and restrictions to secure the <br /> permanent legal existence of the preserved <br /> open space and the proposed wording of any <br /> deed for transfer in fee to the Town or to a <br /> non-profit organization . . <br /> ( f ) A plan indicating the general location of any <br /> proposed well field , sewage treatment plant , <br /> major water or sewer lines , fire hydrants and <br /> any other significant utility element . <br /> (g ) Generalized architectural plans and <br /> landscaping plans for all structures other <br /> than single-family residences , including <br /> treatment of a typical structure , entrance <br /> ways and major signs (w.here development is to <br /> be done in phases , only typical plans are <br /> required at this stage of review) . <br /> ( h ) Copies of the minutes of the pre,-application <br /> conferences held with the , Design Review <br /> Committee , Conservation Commission and Board <br /> -of Health as well as any other letters or <br /> comments by those boards or by the Town <br /> Planner , Superintendent of Streets , Fire <br /> - 18 <br />
