b. Well Locations and Installation <br /> Six wells, in three pairs of shallow (screen set 6 feet below apparent water table) <br /> and deep (screen set 100 feet below surface elevation) wells shall be installed <br /> and maintained as shown on Sheet 4 of the definitive plan entitled "Subdivision <br /> Plan of Land Prepared for CSC Group Located in Mashpee, Mass." dated February <br /> 6, 2001 by Christopher Costa & Assoc., P.O. Box 128 / 465 Main Street, East <br /> Falmouth, Mass., sheets 1-9. <br /> Said wells shall be installed using PVC or other casing approved by the Board of <br /> Health, grouted in place, with a protective locking cover and be otherwise in <br /> conformance with accepted standards for long-term groundwater monitoring <br /> wells. For each well, completion reports and logs, including elevation based on <br /> USGS vertical datum, Massachusetts Coordinate Plane coordinates as well as <br /> surveyed location tied to an established roadway or property bound, depth <br /> ranges of the various soils and sediment layers found in development of each <br /> well shall be provided to the Board, the Local Water Resource Management <br /> Official (Town Planner) and the Board of Health. The Board of Health, Local <br /> Water Resource Management Official and Barnstable County Health and <br /> Environmental Department shall not be denied access to said wells and keys to <br /> any locking covers shall be provided to the Local Water Resource Management <br /> Official. <br /> The applicant shall be responsible for securing, recording and providing to the <br /> Town any necessary easements or permissions to maintain or monitor said wells. <br /> However, monitoring shall continue to be the responsibility of the applicant. <br /> C. Compliance Deadlines <br /> All monitoring wells shall be installed, located correctly on the Subdivision Plan <br /> and all easements shall be secured and presented to the Town prior to any <br /> construction on the site or the issuance of building permits for the project, unless <br /> an extension of time is approved by the Planning Board. No septic system may <br /> be constructed or operated on the site until the above items are completed, and <br /> until the initial sampling described below is completed. <br /> d. Initial Sampling Round <br /> An initial round of water samples shall be extracted from the six monitoring wells <br /> prior to the construction of any septic systems on the site to establish baseline <br /> groundwater quality at the site, and to provide a reference for future monitoring <br /> and interpretation of groundwater quality. The initial sampling round shall <br /> include all constituents contained in the list of parameters in Section 174- <br /> 27(B)(9) of the Mashpee Zoning Bylaw: sodium, iron, manganese, chloroform, <br /> benzene, phosphorous, total coliform bacteria, pH, conductivity, nitrate-nitrogen, <br /> carbon tetrachloride, lead, phenolics, PCB (1254), PCB (1260), <br /> tricholoroethylene, tetrachloroethylene and 11,1,1-trichloroethane. Samples <br /> shall be filtered in the field, held at approximately 4 degrees centigrade and <br /> otherwise be taken in accordance with standard procedures recommended by <br /> -i0- <br />
