MINUTES - BOARD OF HEALTH MEETING, JULY 13, 1989 <br /> page 7. <br /> off tickets have a dollar amount printed on them. The bill total should match the <br /> ticket total . Mr. Greelish indicated the computer he will be bringing in will <br /> utomatically calculate and keep a running total by company and that we need not <br /> Keep a separate ledger for these billings. The computer will also generate the in- <br /> voices. <br /> Mr. Greelish stated that our contract is for a minimum of 7663 tons and a <br /> maximum of 8389 tons. He calculated spreads with 25% and 45% packer contribution. <br /> The cost difference to the Town would be$134,000 - $174,000. He doesn't feel we <br /> will meet our minimum tonnage. The invoice will list the transaction (loop) <br /> numbers. <br /> Mr. Evans stated that we are relying on the honesty of the driver to state <br /> what town the load is from. Spot checking is supposed to take place at Otis. The <br /> driver must sign the drop off slip under penalty of perjury. <br /> 18. APPOINTMENT OF ELIAS A. MCQUAID AS AGENT FOR F/Y 190. <br /> Mr. Greelish motioned to appoint Mr. McQuaid as Agent from July 1 , 1989 to <br /> June 30, 1990; Mr. Doherty seconded, All in favor. <br /> 19. EMERGENCY REPAIR CERTIFICATE - MCDONOUGH <br /> 20. EMERGENCY REPAIR CERTIFICATE - HOWELL <br /> Mr. Evans stated these requests came in this morning and rather than have them <br /> wait another week for the next Board meeting, if the Board would sign off on these <br /> he would investigate further before issuing a repair permit. Both repair requests <br /> came in from Joey DeBarros. Mr. Greelish wants to make sure no additions are <br /> )lanned. Board voted to sign off on emergency repair certificates. <br /> NEW BUSINESS: <br /> Mr. Costa and Mr. Greelish voted to execute the contract with S. Joseph DiMaggio <br /> as operator of the landfill/transfer station, effective July 15, 1989 at 8:00 <br /> am through June 30, 1990 at 4:00pm and proceeded to sign the contract. <br /> Mr. Costa stated that Mr. DiMaggio has met all the requirements of the contract. <br /> Mr. Costa has filed the insurance certificate with the Town Accountant and the <br /> letter of credit with the Town Treasurer. <br /> Mr. Costa stated that the contract is being signed subject to the following <br /> modification of the contract: <br /> "Notwithstanding any other provision to the contrary contained in the contract <br /> document, the commencement date for the contractor is 8:00 am, Saturday, July 15, <br /> 1989. <br /> The contract price to be paid by the owner (town) for the first year of the <br /> contract is reduced by the sum of $7000.00 which sum represents the cost to the <br /> owner to operate the project from July 1 , 1989 through July 14, 1989. <br /> The contractor herein acknowledges that he has inspected the project site <br /> and is familiar with the work performed by the Mashpee Department of Public Works <br /> from Julyl - 14, 1989 and no claim for extra work or funds will be made by the <br /> contractor for any of said work by the D.P.W. <br /> All other terms and conditions of the contract remain in full force and effect. <br /> sated: July 14, 1989 Signed by S.
